• This program is applicable to Medicare Fee-for-Service (FFS) patients only.
  

• MSSP ACOs are attributed Medicare FFS beneficiaries based upon their historical utilization with the ACO's participating providers (based upon provider Tax Identification Number).
  

• We suggest viewing the ACO Summary Fact Sheet available on the portal for language regarding who patients may be attributed to.
  

• An ACO must agree to accept responsibility for at least 5,000 Medicare Fee-for-Service beneficiaries to be eligible to participate in the Shared Savings Program.
  

• The ACO received their beneficiary attribution through the preliminary prospective assignment list and the quarterly assignment list.
